CESA 6 (start up):
  1. Setup Initial User database;
  2. Assist with initial configuration of SEEDS®
  3. Provide initial one-time training per partner group for each of the following: SEEDS®, SBS, Classlist Management, Invoicing and Reports (additional trainings are available at Partner’s expense, see #4 in partner section)
 
CESA 6 (on going):
  1. Maintain secure access
  2. Provide SEEDS® Server
  3. Provide documentation and help desk.
  4. Work with Partner’s contact person to resolve issues and answer questions directly related to SEEDS® system
  5. Organize and facilitate Partners and User Group meetings as needed
  6. Revise, update and disseminate annual user survey to partner’s contact person
  7. Solicit input from Partners regarding enhancements and major updates
  8. Provide all programming
  9. Provide electronic copy of paper forms in Microsoft Word.
  10. Continue to explore enhancements for the system
 Partners (start up):
  1. Prepare and submit initial list of district and school lists
  2. Have technology staff assure that all necessary firewalls are open
  3. Assure that district filters will not block these transactions
  4. Furnish lab and equipment for SEEDS® Forms training
  5. Set up and test training lab including “generic” network login
  6. Assist in initial training
  7. Have technology staff present for troubleshooting before and during trainings
*SBS:
  1. If offered, attend appropriate training(s) from the state (Medicaid) or (EDI).
  2. Apply for Provider Number (www.forwardhealth.gov).  
     
  3. Maintain District user list and levels of permissions to assure and maintain confidentiality as per HIPPA and FERPA.
  4. Apply software upgrades and reinstalls on new or replacement machines as necessary
  5. Train replacement or new staff (Additional training from SEEDS® Team is available at Partner’s expense)
  6. Provide local contact person to resolve issues and/or seek assistance from SEEDS® team
  7. Provide ongoing tech support during the life of the project
 *SBS is a tool. It is the responsibility of the Partner to submit legitimate claims.
