Please share this memo with your principals, PPS staff and other individuals who may initiate referrals to the alternative schools.
Alternative School Process for Consortium EB/D programs:
Procedures to follow prior to sending a student to an alternative EB/D program:
- The current special education teacher contacts their EB/D Program Support Teacher.
- A discussion regarding student progress should take place regarding several items:
a. What are the specific concerns?
b. What interventions have been attempted with what results?
c. How old is the student?
d. If appropriate, what is his/her credit status?
- If an FBA/BIP is not in place, an IEP meeting should be scheduled with the Program Support Teacher to conduct an FBA and develop a BIP.
- If the decision is made to enroll the student in the alternative program, an ANNUAL IEP MUST BE SCHEDULED WITH THE PROGRAM COORDINATOR or PROGRAM SUPPORT TEACHER IN ATTENDANCE.
5. Program Coordinator or Program Support Teacher participates in the IEP meeting. District special education teacher chairs the meeting and authors the IEP with guidance from the Program Coordinator or Program Support Teacher.
6. Sending District Special Education Teacher completes data collection form at the IEP meeting (including exit information from previous program) and submits form to the Program Coordinator or Program Support Teacher, along with the following information:
a. ORGANIZED Pupil Behavioral Record.
b. List of current classes and grades (quarter grades if applicable)
c. Official high school transcript.
d. Health record
e. Copy of free and/or reduced lunch papers (if applicable)
f. Transition plan- district’s required classes (if applicable)
7. LEA arranges transportation for student to attend alternative program.
8. Program Coordinator or Program Support Teacher submits data collection form to Deb Vande Kolk for class list and billing changes.
Students entering the EB/D Alternative Program from NON-Consortium districts:
Non-Consortium Districts are requested to contact Diane Braker, Coordinator of Alternative Programs, 1-800-807-1186 ext.871, 1-920-410-7721 or dbraker@cesa6.k12.wi.us. Diane Braker or her designee will assist the current classroom teacher in completing an annual IEP and RSF-1 form. Prior to entering the alternative program, the Coordinator or designee will want to discuss the following issues with the current EB/D teacher / Director of Special education:
a. What are the specific concerns?
b. What interventions have been attempted with what results?
c. How old is the student?
d. If appropriate, what is his/her credit status?
- If an FBA/BIP is not in place, an IEP meeting should be scheduled with the Program Coordinator or Program Support Teacher to conduct an FBA and develop a BIP.

5. Program Coordinator or Program Support Teacher completes RFS-1 (request for services) and has District Administrator or Director of Special Education from sending district) sign it. The student cannot attend until this form has been completed.
· Program Coordinator or Program Support Teacher submits form into Deb Vande Kolk for entering into database and adding to billing.
